Online or Mail-In Downloadable Declaration Registration Forms

How often I’ve been teaching in the field or even doing teaching at a fireside and everybody goes ‘Where’s a declaration card?’ and we go scouring for one and no one can find one.

Sound familiar? This was the comment from a friend in the Chicago, IL cluster who made the great suggestion of making a declaration (and registration) forms available online that can be easily registered online or downloaded, printed, and mailed in. Here are two options now available online.

A Click-and-Send Option

Many consider this electronic online option more expeditious because the form is not mailed in but registered electronically. By clicking either of the links below, using your computer, smart phone or mobile device you will have access to the official Registration Form to register your declaration, like the card forms available through a Local Spiritual Assembly. 
OR
  • Visit www.bahai.usand click the "I want to be a Bahai" icon to fill out the online electronic form
Once the electronic form is submitted, a Bahai will promptly contact the declarant on the Form to finalize the registration process.

A Download and Mail-In Option

Download and print any of the following Registration Forms whenever you need them. They can also be saved on your personal computer for future use.

  • ENGLISH ADULT DECLARATION FORM
  • ENGLISH JR YOUTH/CHILD REGISTRATION FORM
  • SPANISH ADULT DECLARATION FORM
  • SPANISH JR YOUTH/CHILD REGISTRATION FORM  
  • Once filled out and countersigned, mail the completed Form to:

    Online declaration cards and forms are great but what about not enrolling the new believers until they actually meet with someone in their area in person? We have a fellow who declared online and have yet to have him join in. We've emailed, called, gone to his house, written him...How can one assess the sincerity of anyone through a form?

    We are in receipt of your question about how the online enrollment process works. We are happy to share with you that for each of the declarations that are registered online using the online registration form, there is someone assigned to follow up with that person to ascertain sincerity and to discuss the fundamental verities of the Faith, before the registration is processed. The person assigned may or may not be a person residing within the same community, but it does not mean the process is any less authentic.
